This form is to be completed by a participant AT THE TIME OF ARRIVAL to Old Farm Pool and prior to stepping onto the pool deck. Coaches will assist swimmers with the completion of this form at the time of entry OR swimmers may complete the form themselves from a mobile device at the time of entry.
